Provision of psychological therapy requires the collection of personal data about you and, where appropriate, your family. This is to ensure professional and competent delivery of your therapy including: appropriate sharing of information and liaison with other professionals as necessary; administration; accounting; and invoicing. The lawful basis for collection of such data is that of ‘Legitimate Interest’.
Working Notes, taken during sessions, are written up as a Clinical Record after each session and the Working Notes are then securely destroyed within 3 months. Paper and digital (text and email) records are kept for 7 years (beyond the age of 18 years) to meet legal, professional and indemnity insurance requirements.
If you are unhappy with my handling of your data, in the first instance contact me. I, Karen Green, am the named Data Controller. If your complaint is not resolved to your satisfaction, you can contact the Information Commissioners Office https://ico.org.uk/concerns/handling/ or 0303 123 1113. My data Registration Number is Z3419900
Should there be significant breach in data protection this will be reported to the ICO and yourself within 72 hours once identified.
